Our goal is to make your introduction to our practice as convenient and efficient as possible. To assist us in the endeavor when calling for an appointment as a new patient, please have the following information available:

• Insurance card with information on the type of plan you have (HMO, PPO . . . )
• Your Social Security card or number
• Your Employer information (Name and Address)
• Your primary care or referring physician (Name and Address)

If your insurance plan requires a referral from your primary care physician please be sure the referral is sent to our office prior to your visit.

In order to reduce the necessary paper work on your arrival for your appointment, you can also download the necessary forms and complete them in the comfort of your own home. Please bring your completed forms, your insurance card and a picture ID.
